I bet you’ve been seeing this option ever since you first set up Face ID and have been ignoring it all the time.

This is just what I’m gonna tell you, go set it up.

“Why? I don’t have an alternative appearance…”

Even so, I advise you to do so.

Alternative appearance has two use cases. You want to set it up

  1. if you’re a regular wearer of makeup, sunglasses, or any other thing that might change your appearance, or
  2. if your twin successfully deceived the great Face ID algorithm. (It happened to me. Yes, my sister.)

So, even if you don’t actually have an alternative appearance, setting it up with the same face only makes your iPhone remember your face more clearly i.e., it collects more data of your face.
