What I figured out over the period of time that all these problems are because of lack of consistency, planning, and perseverance. You can successfully complete any task/goal, if you firmly decide to work on it and keep doing it without fail till you reach to the very end of the task/goal. This is as simple and straight forward thing.

But the real problem is we fail to maintain the consistency till the end of the task/goal, thus we fail achieving it. And if you don’t work on this problem now, you will keep on failing in any task/goal in future too. And you will have a hard time achieving the desired result.

(Please note: Your task/goal can be a anything from small daily task of your to-do list to a big monthly/yearly goal.)
