Be aware Alcohol drinking can cause acute pancreatitis!
Pancrease is the most important organ.

There are alot of causes of pancreatitis ,like if I mention those it can be caused by:
gallstone
alcohol
hypercalcemia
hypergylcemia
mumps and coxackie virus
some drugs like azathioprine oral contraceptives.
Autoimmune disorders
but the main culprits are;
two gallstones and alcohol,
Gallstones in gallbladder can cause cholecystitis if that stone slips away from gallbladder and stuck at ampullae of vater then the problem is initiating,
Smaller stone is more dangerous as it can easily get impacted to opening of sphincter
As we know cells in pancrease secret exocrine and endocrine hormones
They have different functions
Exocrine portion of pancrease secrets enzymes and bicarbonate this helps in neutralization of acidic food coming from stomach and help in digestion of food one enzyme is lipase which help in breakdown of lipids and other enzymes breakdown carbohydrates and eventually the breakdown product get absorbed in our circulation,
Endocrine portion the islets of langerhan the beta cells secrets insulin most important hormone it releases due increase blood glucose levels and alpha cells secrets glucagon it releases whenever there is hypoglycaemia , and in stress situations, delta cell secrets somatostatin this hormone inhibit the growth hormone,it’s analogues uses in treatment of gigantism,
So when there is a stone at ampulla of vater basically this opens toward the first part of duodenum the site where food is coming from stomach,
So when the stone get impacted the enzymes will not release into duodenum, accumulation of enzymes in pancrease will activation of enzymes inside the pancrease and there will be autolysis autodigestion and inflammation fibrosis etc, in severe cases enzyme will damage the wall of pancrease this will lead to leakage of enzymes into peritoneum , lipase enzyme will act on mesentric fat and sponification, calcification, calcium levels will get low in the patient, decrease enzyme release can lead to malabsorption but its a late feature, investigation we offer is USG
If alcohol is the cause then it cause the thick secretion because it inhibits the bicarbonate production so thick stagnant secretion will lead to again activation of enzymes and autodigestion and inflammation fibrosis calcification,
Patient presenting complaints! If you have these pay attention, characteristic features are:
Patient will present with,
sudden severe pain in epigaatric region patient will also have pain on the back or towards the right shoulder, It reliefs by bending forward,
Patient will have nausea vomiting etc
As a doctor If it’s a acute case means severe pain then I’ll treat my patient in ICU
- Will keep patient NPO
- Iv fluids
- Analgesics opioids
- Ani inflammatory drugs
After resuscitations will ask for biochemical markers test lipase and amylase
More commonly done is amylase with 24hours if not then lipase is specific if it’s raised 3 folds of normal I’ll label my patient have acute pancreatitis.
If there is doubt because epigastric pain is also a feature of peptic ulcer then
CT scan is confirmatory test in that case
We try to manage patients conservatively because we cannot remove complete pancrease. Only we can do is necrosectomuy of pancrease and endoscopic stenting.
Pancreatitis have severe complications so anyone of you is suffering please take care of yourself avoid fatty foods alcohol.
Systemic and local complications
In systemic ARDS is very important and in local complications peripancreatic fluid collection.
Systemic complications comes first than local.
Chronic condition of this case can cause
- Pancreatitic carcinoma
- Pseudocyst formation
- Severe malabsorption
- Diabetes
